Application for Site Visit (pdf form)
The first step in the verification process.Submit this document with payment to the ABA Central Office; once it is received, we can begin scheduling the visit. Sites will be notified via email when visit dates have been confirmed.
PLEASE NOTE: Application and payment must be received at least four months prior to the requested site visit date.
Effective July 1, 2011
Burn centers that have completed a successful review will
maintain their current expiration date for all future visits. The verification period is still valid for
three years. Example: ABC Burn Center just completed a visit on April 1, 2011; their
current expiration date is June 15, 2011. All future expiration dates will be as follows:
June 15, 2014; June 15, 2017; etc.
This change excludes site visits prior to July 1, 2011.
