Burn Social Worker

Functions as a member of a multi-disciplinary team for Burn Center patients and families in the inpatient and outpatient settings. Assesses and evaluates social determinants of health, identifies needs and barriers, assists in care plan development and goal setting, and identifies and facilitates linkage to network and community resources for patients impacted by complex psychosocial situations.
